We are looking to tell the stories of people who are living without health insurance and who face challenges getting the care they need. We are producing a short film featuring these real stories in order to shed light on this issue and inspire change across the country. We want to tell your story.

• Your family makes less than $50,000 per year (or less than $17,000 per year for a household of one), and
• You live in one these states: Alabama, Florida, Georgia, Kansas, Mississippi, North Carolina, South Carolina, South Dakota, Tennessee, Texas, Wisconsin or Wyoming
